授權及醫療同意

Authorisation & Consent

授權接送人士Authorised Pickup獲准接走孩子的人士姓名及關係。 Names and relationships of people allowed to pick up the child.我明白接送人士在接孩子時或需出示有照片的身份證明I understand pickup persons may be required to show photo ID本人授權本會在孩子參與活動期間，如出現突發身體不適或緊急情況（例如發燒、受傷、過敏反應或其他需要即時處理的健康狀況）時，按實際需要為孩子安排適當的急救或醫療協助，包括聯絡救護車及送院接受治療，並願意承擔相關醫療費用。I authorise the church, during my child's participation in the programme, to arrange appropriate first aid or medical assistance as needed should a sudden illness or emergency arise (e.g. fever, injury, allergic reaction or other health condition requiring immediate attention), including contacting an ambulance and arranging transport to hospital for treatment, and I agree to bear the associated medical expenses.
